The majority of German citizens are in favor of organ donation. Yet less than 0.00115% of Germans are donors, one of the lowest rates in the world, mainly because of their opt-in system.
In fact, there is no official registration. All that is required is a signed statement of intent. Organspendeausweis recommends, "I give permission for organs and tissue to be taken from my body after a doctor has certified my death." And there are no regulations for the appearance of this statement. Which means it could take any form.
